FLU SHOT CLINIC!

Commerce is sponsoring a Flu Shot Clinic on Tuesday, September 27th 2011, from 10:00am- 2:00PM, Education Bldg/5th floor, Room 544. All Health Plan members, including retirees and dependents, will be eligible to receive the vaccine. Employees without the Health Plan coverage will be required to pay $30, cash/check on the day of the clinic. Children under age four (4) MUST visit their physician. Members MUST show a valid BCBSNC ID Card & a Photo ID to receive the Flu vaccine at NO CHARGE. THERE IS NO SEPARATE VACCINE REQUIRED FOR H1NI, IT'S NOW INCLUDED AS PART OF THE STANDARD FLU VACCINATION. For questions call Shakeyia Hazell at 919-715-4204.
